Your Rights and Protections Against Surprise Medical Bills

Under federal law, patients are protected from certain unexpected medical bills through the No Surprises Act.

This notice explains your rights regarding billing protections and good faith estimates for healthcare services provided by Flourish Metabolic ealth (“Practice,” “we,” “our,” or “us”).

1. Good Faith Estimate for Uninsured or Self-Pay Patients

If you do not have health insurance or are not using insurance to pay for services, you have the right to receive a “Good Faith Estimate” explaining the expected cost of your medical care.

This estimate may include:

  • consultation fees,

  • telehealth or e-visit fees,

  • follow-up appointment fees,

  • testing or laboratory-related fees, if applicable,

  • and other reasonably expected healthcare costs.

You may request a Good Faith Estimate before scheduling services or at any time prior to receiving care.

2. Your Right to Receive an Estimate

You have the right to receive a Good Faith Estimate:

  • in writing,

  • at least one business day before a scheduled service if scheduled at least three business days in advance,

  • or at least three business days before a scheduled service if scheduled at least ten business days in advance.

You may also request an estimate before scheduling services.

3. If Your Bill Is Higher Than Expected

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute the bill through the federal patient-provider dispute resolution process.

4. Emergency Services

The Practice does not provide emergency medical services through this website, scheduling platform, or telehealth communications.

If you are experiencing a medical emergency, call 911 or seek immediate emergency medical attention.

5. Insurance and Coverage Limitations

Healthcare coverage and reimbursement vary by:

  • insurance carrier,

  • plan terms,

  • patient eligibility,

  • provider participation,

  • and applicable law.

Patients are responsible for understanding their insurance benefits and verifying coverage where applicable.

The Practice does not guarantee insurance reimbursement or coverage determinations.
